Current Student Forms and Documents

  1. Request for an extension of the time limit. Master’s and Ed.S. students have 6 years to complete their degree programs. Doctoral students have 8 years. Students who anticipate needing additional time should complete the appropriate form and bring to the academic advisor for a signature. The form should be submitted to the Graduate Education Office in the College of Education
  2. Intent to graduate form. All students intended to complete a degree or graduate certificate program must follow an “intent to graduate” form at the beginning of their semester so that a degree check can be done on the program of studies and their names added to the graduate list.
  3. Independent study course outline. All students planning to take an independent study course with a faculty member, typically a course numbered “6497” must complete an outline providing the content of the course, readings, deadlines, means of evaluation, etc. Ideally, this should be done before the semester begins, but at the latest during the first week of the semester.
  4. Doctoral Student Application for Pre-Candidate Status. Soon after admission to the doctoral program, a doctoral student should meet with his or her advisor and faculty advisory committee to agree on the program of study. This program of study is then entered on an Application for Pre-Candidate Status. The forms requires the signatures of committee members and then should be submitted to the Graduate Education Office in the College of Education
