Academic Program - Helpful Forms and Documents

 

Important Dates

From this page you can link to forms and to other helpful documents.  There are two categories of forms:  those designed and numbered by the Graduate School for all the graduate students on campus; and those designed and numbered by the College of Education just for the graduate students in Education.

Current Student Forms and Documents

I. Master of Education (M.Ed.) Students

Forms Required of All Students

  • Advisor Assignment Form - Office of Advising and Student Services Graduate Student Faculty Advisor Assignment Information 
  • Program of Study (M-1):  Students use this form to list all of the required and elective courses comprising their degree.  Courses should be selected with the faculty advisor.  This is a Graduate School form so click on the link above.
  • Master's Intent to Graduate (M-4):  Students submit this form at the beginning of their last semester to notify us that they intend to graduate at the end of the semester.  This initiates a degree check and adds student names to the graduation list.

Special Request Forms

  • Master's Time ExtensionMaster's students are given 6 years to complete their Master's degree programs.  If students find they need additional time they can request it by completing this form, obtaining the advisor's signature, and submitting the form to the Graduate Education Office.
  • 6497 Independent Study or "Problems” Course Outline:  Students are allowed a limited number of credit hours of independent study courses typically numbered 6497.  Students must complete this outline in consultation with the supervising faculty member.  It is an agreement of what tasks are to be completed by specified dates and what work the instructor will evaluate (grade).  This form should be submitted to the division office of the curricular designation (Ed Adm, Ele Ed, etc.) to receive a consent number to register.
  • Change of Program Form (G-8): Students use this form to change their M-1s. Obviously, required courses cannot be changed, but elective courses can be added and dropped from the program of study. This is a Graduate School form and it can be downloaded by clicking here.

II. Education Specialist (Ed.S.) Students

Forms Required of All Students

  • Program of Study (S-1):  Students use this form to list all of the required and elective courses comprising their degree.  Courses should be selected with the faculty advisor.  This is a Graduate School form so click on the link above.
  • Ed.S. Intent to Graduate (S-4):  Students submit this form at the beginning of their last semester to notify us that they intend to graduate at the end of the semester.  This initiates a degree check and adds student names to the graduation list.

III. Graduate Certificate Students

Forms Required of All Students

  • Program of Study (M-1): Students use this form to list all of the required and elective courses comprising their certificate.  Courses should be selected with the faculty advisor.  This is a Graduate School form so click on the link above.
  • Graduate Certificate Intent to Complete Program (M-4): Students submit this form at the beginning of their last semester to notify us that they intend to complete certificate studies at the end of the semester.  This initiates a completion check and adds the certificate to the student transcript at the end of the semester.

IV. Doctoral Students

Forms Required of All Students

  • Program of Study/Application for Pre-Candidate Status: This form lists all of the Master's and doctoral courses used to satisfy the required of the appropriate doctoral degree.  It is completed about one year after admission in consultation with the advisor and the advisory committee.  It should be submitted to the Graduate Education Office after approval by the advisor and advisory committee.
    1. Doctor of Education (ED-2) (Optional Word Version)
    2. Doctor of Philosophy (ED-3) (Optional Word Version)
  • Change of Doctoral Program Advisor:  This form is used if it is necessary to change the doctoral program advisor assigned at admission.
  • Intent to do Residency (ED-4):  This form is completed by students to indicate what courses taken over the specified time frame satisfy a residency requirement.
  • Appointment of Comprehensive Examination Committee:  This is a Graduate School form normally completed by faculty.  It can be downloaded by clicking here).
  • Comprehensive Examination Committee Report (ED-5):  This form is normally completed by faculty to report the outcome of a comprehensive examination.
  • Application for Candidacy:  This is a Graduate School form and it can be downloaded by clicking here).
  • Forms Related to the Dissertation:  Forms related to the dissertation committee and submission of dissertation proposals and dissertation drafts are Graduate School forms and can be downloaded by clicking here.
  •  Declaration of Intent to Graduate (ED-6) 
  • Graduate School Doctoral Forms (D1 to D10)

Special Request Forms

  • Doctoral Time Extension: Doctoral students are allowed 8 years from the time of their first post-Master's course or first post-Ed.S. course to complete the doctoral degree. If students find they need additional time they can request an extension of their time limit by completing this form.
